About Harbor Health Services
At Harbor Health, the patient, his/her family and the community are central to what we do. We provide comprehensive services including pediatric, family and adult medicine, elder care, dental services, and behavioral health. Whether it is a routine annual physical exam or ongoing treatment of a chronic condition, we are here for you. At Harbor Health, we go beyond the ordinary. Our patients and the communities we serve have access to: laboratory services, optometry and podiatry, home health as well as primary and rehabilitative care for elders, community based programs focusing on youth and issues that impact the overall health of the community, health and nutrition education, social and interpreter services, as well as financial counseling and assistance enrolling in health care coverage.
All Harbor Health physicians, nurse practitioners and dentists are board eligible or board certified in their specialty, and trained at some of the most respected medical facilities in the United States and abroad. Our physicians, nurses, specialists and support staff work as a team to insure an integrated and comprehensive approach for all our patients providing them the necessary tools to live healthier lives.

The Elder Service Plan
The Elder Service Plan of Harbor Health Services, Inc. is a PACE (Program of All-Inclusive Care for the Elderly) program. We are committed to providing quality comprehensive care to those meeting the eligibility criteria, using an interdisciplinary health care team approach.
