WHAT IS THE PATIENT PORTAL?
The Harbor Health Services, Inc. Patient Portal is an easy way to get in touch with your care team. It is a secure application that allows you to: email your Care Team directly, renew your prescriptions, receive information from your Care Team and research health topics.
WHY SHOULD I USE THE PATIENT PORTAL?
Let’s face it: getting into contact with your Care Team can be challenging. Harbor’s Patient Portal can help. If you have a non-urgent medical question, you can message your Care Team through the Patient Portal at any time of day or night. We will respond to your message within 1 business day.
Additionally, the Patient Portal has a research center full of information that can be very helpful to you. Here, you can check your symptoms and learn about different health topics – both in English and Spanish.
HOW DO I KNOW MY HEALTH INFORMATION IS SECURE?
Harbor has made deep investments to secure your information. Data sent via the Patient Portal is encrypted and 100% secure.
HOW CAN I VIEW MY LAB RESULTS?
One of the newest features is the ability to view a copy of your most recent medical record. However, this information does not upload automatically and must be requested from the health center. Send a secure message from your Patient Portal account asking your healthcare provider for the status of a recent lab result. When the provider sends the electronic copy of the lab result to your account, you will receive an email to let you know a document is ready in your Inbox. You can also ask for your entire health record which has your lab results by clicking “Request Health Record” from under the “My Chart” tab.
WHAT IF I DON’T CHECK MY EMAIL OFTEN?
Don’t worry, you now have the option to receive any notifications from the Patient Portal via text messaging if that is more convenient for you. Simply provide your cell phone number and carrier on the Patient Portal enrollment form and we will set you up to receive text notifications.

WHAT IS THE 8-DIGIT SECURITY TOKEN?
This is an 8-digit number that is generated for security purposes when initially enrolling into the Patient Portal. It can also be used to reset your password. Lucky for you, you do not need an 8-digit security token. Harbor Health takes care of the initial enrollment. When signing up for the Portal, the information you provide is used to enroll and set up your account for you. A username and temporary password will be given to you to access your account that is ready to use!
IF I SIGN UP FOR THE PATIENT PORTAL, CAN I STILL CALL THE HEALTH CENTER TO TALK TO MY DOCTOR OR MAKE APPOINTMENTS?
Of course! In fact, the Patient Portal is not to be used for urgent or emergency matters. For any questions that require an immediate response, you must call the Health Center and speak to a member of your Care Team.
MY USERNAME IS NOT WORKING. I TYPED MY EMAIL ADDRESS CORRECTLY. WHY ISN’T IT WORKING?
Generally speaking, your username is the beginning half of the email address you used to sign up for the Patient Portal. For example, with the email address firstname.lastname@example.org, the username would be patientportal. Remember to drop the @___ when signing into your account.
Be sure to type your username and password carefully. Both the username and password are case sensitive.

I FORGOT MY PASSWORD WHAT SHOULD WE DO?
With the busy lives we lead, it is not uncommon to occasionally forget some of the many things we have to remember. To reset your Patient Portal password:
- Open the Patient Portal login page (www.nextmd.com).
- Click on the link that says “Need help with your user name and password?” In the first box, type in your username.
- Click “Next” on the bottom of the page.
- Answer your security question.
- A link to choose your new password will be emailed to you. Still stuck? On the same page, enter your name, email address, date of birth, and zip code.
- Click “Next.”
