The Harbor Health Elder Service Plan (PACE) is a fully integrated healthcare model providing a comprehensive service package delivered by a caring group of healthcare professionals who know each participant and their care partner’s givers personally. PACE provides participants and caregivers the healthcare and support needed, to ensure older adults with chronic medical needs can remain at home, independent and healthy with their loved ones.
PACE began in the late 70’s in California, today two hundred and forty PACE programs serve almost 100,000 participants in 19 states across the country. Massachusetts is fortunate to have 8 programs. PACE is a program of Medicare and Medicaid.
Older adults age 55 and up are eligible*. PACE provides healthcare for elders with chronic conditions having difficulty living independently without support services. PACE members are suitable for nursing home level of care but can live at home safely with additional PACE medical and social supports. PACE members need to reside in our service area.
To qualify for the program without a monthly premium monthly gross income should be below Medicaid guidelines. Medicaid does have some limitations on assets.
Those with income above Medicaid level may qualify for PACE with a monthly premium.
*Between ages 55 to 65 you must be disabled, over age 65 disability not required.
All medical benefits and services offered by Medicare and Medicaid (MassHealth) are included in the PACE benefit package. The benefits are provided and/or coordinated by the dedicated PACE interdisciplinary team working in coordination with participants and their care partners. Covered services are offered when medically necessary. These benefits are all covered in full.

You are invited to schedule a tour to visit the PACE Center or we can meet with you at your home. Also, a PACE nurse will meet with you and your care partners to better understand your medical needs and see if PACE would be a good option for your care.
In addition, the PACE team will complete the Medicaid application on your behalf. Throughout the enrollment process you will have a team member available to answer all your questions.
