To make an appointment, please contact the location most convenient for you and tell them you are a new patient. When you call, we will help you decide if a video/phone or in-person appointment is best for your care needs and let you know what information we will need for new patient registration.Find a Location
How Do I Register As A New Patient?
Nothing should stand in the way of your health! That’s why we make becoming a new patient as easy and fast as possible for all of our new and first-time patients.
If you are a new patient, you can download, print, and complete the registration forms below before your first appointment. Completing the form ahead of your appointment ill allow you to check-in FASTER.
If you have any questions about the registration forms, please contact the Harbor Health location where you have your appointment.
Please check with your health insurance company before your first appointment to make sure your visit to Harbor Health is covered by your plan.
Do You Need Help Applying for Health Insurance?
Harbor Health can help you apply for MassHealth (Medicaid) and public insurance programs. When you call to make an appointment, let us know you will need help applying for insurance.
