Behavioral Health

Fully integrated into the medical setting, Behavioral Health clinicians work with primary care teams to support patients whose medical needs are affected by social and/or behavioral concerns. Clinicians consult with medical providers and provide direct services to patients, including: assessments, brief interventions, coaching, screenings, follow-up visits during medical appointments and recommendations and referrals to long-term services.

Additional services are available at certain sites. At Daniel Driscoll – Neponset Health Center and Geiger Gibson Community Health Center, there are clinicians available who provide short-term mental health therapy. Furthermore, psychiatric services are available at Daniel Driscoll – Neponset Health Center. These services include psychiatric evaluations after an intake is completed with a therapist and medication management when appropriate. Substance abuse counseling is available at Geiger Gibson Community Health Center, Harbor Community Health Center – Plymouth, and Harbor Community Health Center – Hyannis.
