Online Patient Registration Forms

Harbor Health Services, Inc. provides Online Patient Registration forms which may require confidential, personal, and health related information. For the protection of our patients, there is no option to fill out the forms on-line. We ask that all new patients download, print and fill out the forms before coming to their scheduled appointment.

Email Communications

Our site offers a mechanism to contact us with general questions. Please note that your email, like most Internet email messaging services, does not provide a secured method of delivery for you to communicate with us and other third parties. It is possible that your email communication, if not encrypted, may be accessed or viewed inappropriately by another Internet user while in transit to us. If you wish to keep your information completely private, you should not use email to contact us.

Harbor Health does not collect your email address unless you voluntarily submit it to us or choose to communicate with us via email. We do not sell or rent any email addresses. We try to respond to email messages requiring a response within two business days. If you use the “contact us” page to communicate with us, your message and email address will be forwarded to the appropriate division within the organization for follow up. If you are a patient, we encourage you to use our on-line “customer satisfaction survey” tool, when you do that, your message and email address will be forwarded to the Executive Director of the division where you are receiving services. Please do not email patient health information (PHI), as that information is confidential and should only be discussed with your physician.

We may send email communications to you related to any changes in our services, hours of operation, any organizational updates, our electronic newsletter and other general health topics that maybe of interest to you.
