Services

Meetings with patients may be up to 30 minutes for assessment, brief interventions, coaching, screenings, follow up visits in primary care, recommendations and referrals to other services. These visits may be billed to insurance companies.

Meetings with primary care teams, as requested, to consult on how to work more effectively with patients whose medical
conditions are affected by social or behavioral problems.

Goals of Services

Support patients and their primary care teams in promoting and improving physical and emotional health by working as a team and providing more holistic care. The primary care teams and the health consultants share electronic health records for better communication and coordination.
